Online Prescription Pick up

This form is for patients WHO HAVE ALREADY ordered their prescription with their Surgery (for example by phone, taking in your repeat slip or using your surgery's online facility) and just require us to pick up their prescription.  We will then pick up your prescription and have it ready for you to collect from the pharmacy or delivery it if you are housebound or elderly.

Please note you can also use our secure online ordering facility to order your medication and we will request your medication for you.  If you prefer to use a a secure facility to order your prescription pick up then use our online ordering facility and write the day you would like us to collect your medication in the medication line.
